Background
Toll-like receptor 4 (TLR4) is a type I transmembrane pattern-recognition receptor expressed predominantly on macrophages, dendritic cells, and epithelial barriers; upon recognition of lipopolysaccharide from Gram-negative bacteria in complex with MD-2 and CD14, it homodimerizes and initiates a MyD88- and TRIF-dependent signaling cascade that activates NF-κB, MAPKs, and IRF3, driving the transcription of pro-inflammatory cytokines (TNF-α, IL-1β, IL-6), type I interferons, and co-stimulatory molecules essential for innate immunity and the subsequent shaping of adaptive responses, while excessive or dysregulated TLR4 activation contributes to septic shock, chronic inflammatory disorders such as atherosclerosis and metabolic syndrome, and ischemia–reperfusion injury, making it a therapeutic target for antagonists like lipid A analogs and monoclonal antibodies.
