Background
Interleukin-18 (IL-18) is a 18 kDa pro-inflammatory cytokine of the IL-1 superfamily, constitutively expressed by macrophages, dendritic cells, and epithelial cells as an inactive 24 kDa pro-peptide that is cleaved by caspase-1 within the inflammasome to its mature form; once secreted, IL-18 binds the heterodimeric receptor IL-18Rα/IL-18Rβ and recruits the adaptor MyD88 to activate NF-κB and MAPK pathways, synergizing with IL-12 to drive IFN-γ production from Th1 cells and NK cells and promote cell-mediated immunity, but when unchecked it contributes to the pathogenesis of autoimmune diseases such as rheumatoid arthritis, inflammatory bowel disease, and type 1 diabetes, making it both a biomarker and therapeutic target.
