Background

The interleukin-6 receptor (IL-6R/CD126) is a type I cytokine receptor existing in membrane-bound and soluble forms. The functional receptor complex comprises the ligand-binding IL-6R subunit and the signal-transducing glycoprotein 130 (gp130). IL-6 signaling occurs through two distinct pathways: classic signaling via membrane-bound IL-6R and trans-signaling mediated by the soluble IL-6R (sIL-6R), which is generated through proteolytic cleavage. This trans-signaling mechanism enables IL-6 to act on cell types lacking membrane-bound IL-6R by forming complexes with sIL-6R that activate gp130. The IL-6R system regulates immune responses, cell growth, and differentiation, but its dysregulation is implicated in various chronic inflammatory diseases, autoimmune disorders, and malignancies. Consequently, therapeutic targeting of this pathway, particularly through humanized anti-IL-6R monoclonal antibodies, represents a promising approach for treating IL-6-driven pathologies.
