Background
The Farnesoid X receptor (FXR, NR1H4) is a ligand-activated nuclear receptor that functions as the master bile-acid sensor: after binding physiological bile acids such as chenodeoxycholic acid, FXR heterodimerizes with RXR, translocates to DNA FXR-response elements and trans-activates or represses genes that control bile-acid synthesis (CYP7A1), conjugation (BACS, BAAT), export pumps (BSEP, MRP2) and intestinal re-uptake (IBABP, ASBT), while additionally regulating lipid and glucose metabolism, hepatic lipogenesis, energy balance, inflammatory signaling, gut barrier integrity and innate immunity; FXR exists as four human isoforms (α1–α4) and is expressed mainly in liver, intestine and kidney, with synthetic agonists (e.g. GW4064, obeticholic acid) and antagonists under intense investigation for treating cholestasis, NASH, fibrosis, diabetes and intestinal inflammatory disorders.
