Background
The interleukin-4 receptor alpha (IL-4Rα/CD124) is a crucial 140 kDa transmembrane glycoprotein that functions as the primary signaling component for IL-4. It forms two distinct receptor complexes: the type I receptor with the common gamma chain (γc) specific for IL-4, and the type II receptor with IL-13Rα1 that responds to both IL-4 and IL-13. These receptors mediate key immunological processes including Th2 cell differentiation, alternative macrophage activation, B-cell class switching, and allergic inflammation. The receptor's structure features an extracellular cytokine-binding domain, a transmembrane segment, and a cytoplasmic region containing Box 1 and ITIM signaling motifs. Soluble forms of IL-4Rα generated through alternative splicing or proteolysis can inhibit IL-4 activity by competing for ligand binding. Given its central role in type 2 immunity, IL-4Rα has emerged as an important therapeutic target for allergic diseases such as asthma and atopic dermatitis, with targeted biologics demonstrating clinical efficacy by blocking both IL-4 and IL-13 signaling pathways.
