Background

EpCAM (also known as GA733-2 antigen, CO171A, EGP40, TROP1, etc.) is a type I transmembrane glycoprotein. Its structure consists of an extracellular domain featuring two EGF-like repeats and a cysteine-rich region, a single transmembrane domain, and a short cytoplasmic tail.EpCAM functions as a calcium-independent homophilic cell adhesion molecule. Beyond mediating cell adhesion, it actively regulates cell proliferation, migration, and differentiation. Its intracellular domain can be proteolytically released and translocated to the nucleus, where it exerts oncogenic signaling functions.EpCAM is intricately linked to key signaling pathways. It cross-talks with the N-cadherin/Pi3K axis, leading to disruption of cadherin-based adhesion complexes. Moreover, EpCAM is a direct transcriptional target of the Wnt/β-catenin pathway and itself signals through elements of this pathway, contributing to intracellular signaling and cell polarity.As a pan-epithelial differentiation antigen, EpCAM is overexpressed in most carcinomas. Its overexpression is associated with high tumor stage, high grade, and poor prognosis in multiple cancer types, including breast cancer. It serves as a well-established tumor marker and a promising target for molecular therapies (e.g., monoclonal antibody-based treatments).
