Background

Symmetric Dimethylarginine (SDMA) is a methylated derivative of the amino acid arginine, formed through post-translational modification of proteins. Historically considered inactive, it is now primarily recognized as a highly sensitive and specific endogenous biomarker for renal function, particularly glomerular filtration rate (GFR). SDMA is freely filtered by the glomeruli and is not secreted by the renal tubules, making it a more accurate indicator of renal filtration than serum creatinine, as it is less influenced by non-renal factors such as muscle mass, diet, or sex. Its measurement is increasingly valuable in the early detection and monitoring of both acute kidney injury and chronic kidney disease in human and veterinary medicine.
