Background
URAT1 (Urate Transporter 1), encoded by the SLC22A12 gene, is a critical transmembrane protein predominantly expressed on the apical membrane of proximal tubule epithelial cells in the kidney, where it functions as the primary mediator of renal urate reabsorption by facilitating the exchange of intracellular dicarboxylates or monocarboxylates for luminal uric acid. As a key determinant of serum uric acid homeostasis, URAT1 activity directly influences circulating urate levels, and loss-of-function mutations in this transporter are causally linked to idiopathic renal hypouricemia, whereas its overactivity contributes to hyperuricemia and gout pathogenesis. Consequently, URAT1 has emerged as a major pharmacological target for uricosuric therapies, with drugs such as benzbromarone, probenecid, lesinurad, and dotinurad specifically designed to inhibit this transporter’s function, thereby enhancing urinary uric acid excretion and lowering serum urate concentrations in patients with gout and hyperuricemia.
