Background

Transient Receptor Potential Melastatin 4 (TRPM4) is a calcium-activated, non-selective cation channel belonging to the melastatin subfamily of TRP channels that plays a pivotal role in regulating membrane potential and cellular excitability across various tissues, including the heart, smooth muscle, immune cells, and neurons. Unlike other TRP channels, TRPM4 is impermeable to calcium ions despite being strictly activated by intracellular calcium rises, thereby functioning primarily to depolarize the cell membrane by allowing the influx of sodium and potassium ions, which subsequently modulates voltage-gated calcium entry and downstream signaling pathways. Structurally, it forms tetrameric complexes with six transmembrane domains and is subject to complex regulation by phosphatidylinositol 4,5-bisphosphate (PIP2), ATP, and phosphorylation, ensuring precise control over its activity; physiologically, TRPM4 is critical for cardiac action potential duration and rhythm stability, vascular tone regulation, insulin secretion in pancreatic beta-cells, and immune responses such as T-cell activation, making it a significant therapeutic target for cardiovascular diseases, metabolic disorders, and inflammatory conditions.
