Background
Transthyretin/Prealbumin is a 55-amino-acid, homotetrameric, β-sheet-rich transport protein synthesized primarily in liver and choroid plexus that binds thyroxine (T4) and retinol-binding protein–vitamin A complexes, serving as the major thyroid-hormone carrier in cerebrospinal fluid and a key contributor to retinol homeostasis in plasma; its small size, short half-life (~2 days), and negative acute-phase behavior make serum levels a sensitive indicator of hepatic protein synthesis and nutritional status, while point mutations or aging-associated tetramer destabilization can trigger misfolding and pathogenic extracellular amyloid fibril deposition in familial and senile systemic amyloidosis, respectively.
