Background

Tau protein is a core biomarker and therapeutic target for Alzheimers disease (AD). Abnormal hyperphosphorylated tau forms neurofibrillary tangles, a key pathological hallmark of AD. Cerebrospinal fluid and blood phosphorylated tau isoforms enable early, accurate AD diagnosis, differentiating it from other dementias. Tau PET imaging visualizes its brain deposition, tracking disease progression and predicting cognitive decline. For treatment, tau has become a major therapeutic focus. Anti-tau antibodies, aggregation inhibitors and modulators of tau phosphorylation are actively developed to block tau spreading and reduce pathology. Overall, tau supports early screening, differential diagnosis and disease monitoring, while driving the development of novel targeted therapies for AD.
