Background
Triiodothyronine (T3) acts as the biologically potent thyroid hormone, exerting profound effects on basal metabolic rate, protein synthesis, and neural development. Although secreted in smaller quantities by the thyroid gland compared to Thyroxine (T4), T3 possesses significantly higher metabolic activity, with the majority circulating in the blood derived from the peripheral deiodination of T4. The diagnostic assessment of T3 is integral to the comprehensive evaluation of the hypothalamic-pituitary-thyroid axis. Its measurement is particularly crucial for the early detection of hyperthyroidism, where it often serves as a more sensitive indicator than T4. Furthermore, T3 quantification aids in distinguishing between thyroidal and extrathyroidal illnesses; for example, low T3 levels are a hallmark of "Euthyroid Sick Syndrome" in non-thyroidal systemic illnesses. Thus, T3 remains a cornerstone biomarker in the endocrine workup of metabolic and thyroid disorders.
