Background

SV2A, a synaptic vesicle glycoprotein, serves as a promising synaptic density biomarker for Alzheimer’s disease (AD) diagnosis. In AD, SV2A levels in cerebrospinal fluid (CSF), serum, and brain tissue decline significantly, correlating with cognitive impairment and disease progression. SV2A PET enables in vivo synaptic loss visualization, offering early detection of AD-related brain dysfunction before obvious atrophy. CSF/serum SV2A strongly correlates with PET results, providing minimally invasive alternatives for synaptic assessment.

Notably, serum SV2A shows high sensitivity (97.8%) for identifying amnestic mild cognitive impairment (aMCI), outperforming NfL, GFAP, and p-tau217 in early AD detection. It also effectively distinguishes high-risk individuals from cognitively unimpaired populations.

As a key synaptic integrity marker, SV2A aids early diagnosis, progression monitoring, and risk stratification in AD, complementing existing pathological biomarkers.
