Background
CD138 (syndecan-1) is a type-I transmembrane heparan-sulfate/chondroitin-sulfate proteoglycan that tethers the cytoskeleton to the extracellular matrix, mediating adhesion, migration, and exosome biogenesis while acting as a co-receptor for growth factors and chemokines . It is constitutively expressed on epithelial cells and terminally differentiated plasma cells (making it a selective marker for plasma-cell neoplasms), but absent from mature B cells. Shed soluble CD138 is a prognostic indicator in multiple myeloma and other cancers; its overexpression or loss correlates with tumor progression, angiogenesis, and metastasis in solid tumors such as breast, colorectal, and liver cancers .
