Background

Povetacicept (ALPN-303) is an engineered Fc fusion protein consisting of a variant domain of human TACI receptor (containing key amino acid substitutions) fused to an effector-less human IgG1 Fc fragment, functioning as a dimer to exert high-affinity dual antagonistic activity. Its mechanism of action is achieved through simultaneous binding and inhibition of B-cell activating factor (BAFF) and a proliferation-inducing ligand (APRIL)—the former regulates early B-cell development and activates T cells, while the latter supports plasma cell survival, both collectively driving pathogenic B-cell and autoantibody production. Clinical studies (RUBY-3) have demonstrated that this agent reduces proteinuria by 64% and galactose-deficient IgA1 by 77% in IgA nephropathy, with approximately 90% of patients achieving resolution of hematuria; in membranous nephropathy, anti-PLA2R antibodies decreased by 83%, with 100% reaching immunological remission, and the safety profile has been favorable. Currently, a Phase III confirmatory study (RAINIER) has been initiated in IgA nephropathy, and the drug has also shown therapeutic potential in other autoantibody-associated diseases such as myasthenia gravis.
