Background
Pancreatic stone protein (PSP) is a circulating C‑type lectin mainly secreted by pancreatic and intestinal tissues, serving as a key inflammatory biomarker for systemic infection and critical illness. Its serum concentration elevates far earlier than conventional markers such as PCT and CRP, often rising up to 72 hours before clinical deterioration of sepsis, enabling early risk recognition in ICU populations. PSP exhibits favorable performance for identifying sepsis‑related conditions, correlating closely with SOFA scores and disease severity. It helps stratify mortality risk for septic patients, community‑acquired pneumonia and sepsis‑triggered ARDS, showing higher diagnostic accuracy than procalcitonin in several cohorts.
As a damage‑associated molecular pattern, PSP reflects magnitude of host inflammatory response rather than specific pathogen types. It supports early warning for hyper‑inflammatory phenotypes and assists antibiotic stewardship by monitoring treatment response. Nevertheless, PSP cannot reliably distinguish infectious from non‑infectious systemic inflammation. Combined usage with other biomarkers improves overall diagnostic power. Further large‑scale validations are required to standardize cut‑off values and consolidate its routine clinical application in IVD assays.
