Background
NOTCH3 is a single-pass transmembrane receptor belonging to the Notch family, which plays a crucial role in cell fate determination, differentiation, and vascular development through the canonical Notch signaling pathway. Predominantly expressed in vascular smooth muscle cells (VSMCs) and pericytes, NOTCH3 regulates the maturation, survival, and function of these cells, thereby maintaining the structural integrity and tone of small arteries and arterioles. Upon binding to its ligands (such as Jagged or Delta-like proteins) on adjacent cells, NOTCH3 undergoes proteolytic cleavage, releasing its intracellular domain (NICD3) to translocate to the nucleus and activate target gene transcription. Mutations in the NOTCH3 gene are the established cause of Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy (CADASIL), a hereditary small vessel disease characterized by recurrent strokes, migraine with aura, and progressive cognitive decline, underscoring its critical importance in cerebrovascular health.
