Background

Complement component 3 (C3) is the central effector molecule of the complement cascade, participating in classical, lectin, and alternative activation pathways. Circulating serum C3 is routinely measured to evaluate complement system activation status in clinical and pre‑clinical settings. Decreased C3 levels typically indicate excessive complement consumption, commonly seen in systemic lupus erythematosus, membranoproliferative glomerulonephritis, sepsis, and some autoimmune vasculitis. In contrast, elevated C3 often acts as an acute‑phase reactant, rising during bacterial infection, trauma, and chronic inflammatory disorders.

C3 testing aids differential diagnosis, disease activity monitoring, and treatment response assessment for immune‑mediated diseases. It helps stratify patient risk for complement‑driven tissue injury. However, C3 concentration lacks high specificity. Low C3 cannot pinpoint exact activation pathways, and normal C3 does not exclude local complement activation within tissues. Therefore, C3 should be interpreted alongside C4, complement split products such as C3a and iC3b, and clinical manifestations. Combined biomarker panels improve diagnostic confidence for IVD and translational research.
