Background

Matrix Gla Protein (MGP) is a vitamin K-dependent, non-collagenous protein primarily synthesized by vascular smooth muscle cells and chondrocytes that serves as the most potent known endogenous inhibitor of ectopic calcification in soft tissues. Functioning through a mechanism dependent on vitamin K-mediated gamma-carboxylation of specific glutamic acid residues, activated MGP binds to calcium crystals and bone morphogenetic proteins to prevent hydroxyapatite deposition in arterial walls and cartilage, thereby maintaining vascular elasticity and preventing pathological mineralization. Its critical physiological role is underscored by the fact that loss-of-function mutations in the MGP gene cause Keutel syndrome, a rare autosomal recessive disorder characterized by abnormal cartilage calcification and peripheral pulmonary artery stenosis, while insufficient vitamin K status or warfarin therapy can lead to undercarboxylated, inactive MGP accumulation, which is strongly associated with increased risks of vascular calcification, cardiovascular disease, and osteoarthritis progression.
