Background

MC5R, a 325-amino-acid G-protein-coupled receptor encoded by MC5R on chromosome 18p11.2, is the most promiscuous of the five melanocortin receptors, binding α-, β- and γ-MSH as well as ACTH with nanomolar affinity to activate Gαs/cAMP signaling; it is highly expressed in sebaceous, lacrimal, meibomian and preputial glands, driving lipid-rich secretions that waterproof skin and hair, modulate thermoregulation, scent marking and innate immunity, and its global or sebocyte-specific deletion in mice produces a severe “dry coat” phenotype, altered sebum lipidomes, increased transepidermal water loss and susceptibility to dermatitis, while in humans MC5R polymorphisms correlate with acne severity and seborrheic dermatitis, making it a promising, highly tissue-restricted target for next-generation topical or systemic therapeutics aimed at controlling sebum output, dry eye, androgen-independent acne, and even metabolic syndrome through its recently discovered roles in adipose tissue lipolysis and hepatic steatosis.
