Background

Lrg1 protein (Leucine-Rich Alpha-2-Glycoprotein-1) is an evolutionarily highly conserved secreted glycoprotein that belongs to the leucine-rich repeat (LRR) protein family. Its core structure is composed of eight LRR motifs, supplemented with glycosylation sites and disulfide bonds, which efficiently mediate protein–protein interactions. Functionally, the most critical mechanism of Lrg1 is its direct binding to co-receptors (such as Endoglin) in the transforming growth factor-β (TGF-β) signaling pathway. Under pathological conditions, it "switches" the TGF-β signal from the anti-inflammatory Smad2/3 pathway toward the pro-angiogenic Smad1/5/8 pathway, thereby potently driving aberrant angiogenesis. Although Lrg1 is largely redundant in normal development and physiological homeostasis (Lrg1-knockout mice show no obvious abnormalities), its expression is significantly upregulated under stimuli such as inflammation and hypoxia, and it plays a pathogenic role in tumor growth, diabetic retinopathy, cardiovascular diseases, and various inflammatory disorders. Moreover, because its expression levels dynamically change with disease states and can be detected in peripheral blood, Lrg1 is also widely regarded as a promising biomarker for disease diagnosis, prognosis evaluation, and disease monitoring.
