Background
Leucine-rich repeat-containing G protein-coupled receptor 6 (LGR6) belongs to the LGR4/5/6 subfamily and serves as a receptor for R-spondins (RSPO1–4) and for the pro-resolving lipid mediator maresin 1 (MaR1). Structurally, LGR6 is a class A G protein-coupled receptor with seven transmembrane helices and a large extracellular N-terminal domain containing multiple leucine-rich repeats (LRRs). R-spondin binding to this ectodomain potentiates canonical Wnt/β-catenin signaling by cooperating with RNF43/ZNRF3, whereas MaR1 engagement activates Gs/cAMP-dependent signaling. LGR6 marks adult epithelial stem cells in the skin, lung, mammary gland, kidney, intestine, taste buds, and nail, contributing to tissue homeostasis, injury repair, and inflammation resolution; it also exerts protective effects in the heart, cartilage, and intervertebral disc. Clinically, LGR6 is used as a stem-cell marker in regenerative medicine and is recognized as a prognostic biomarker and potential therapeutic target in cervical, triple-negative breast, lung, pancreatic, and esophageal cancers. The MaR1–LGR6 axis represents a promising intervention point for diabetic cardiomyopathy, osteoarthritis, intervertebral disc degeneration, and ischemia-reperfusion injury, while RSPO–LGR6-dependent Wnt signaling is an active area of drug development.
