Background
The α subunit encoded by the KCNMA1 gene is the core pore-forming protein that constitutes the large-conductance calcium-activated potassium channel (BK channel, also known as MaxiK or KCa1.1). This protein has a molecular weight of approximately 134 kDa and belongs to the voltage-gated potassium channel superfamily. Its unique feature lies in its ability to respond simultaneously to membrane depolarization and increases in intracellular calcium concentration, mediating potassium efflux to rapidly repolarize or hyperpolarize the cell membrane, thereby precisely regulating the excitability of various excitable cells, including neurons and smooth muscle cells. Structurally, four identical subunits of the KCNMA1 protein assemble into a homotetramer, with each subunit containing seven transmembrane segments (S0–S6) and a large intracellular C-terminal domain. The S4 segment serves as the voltage sensor, while the C-terminus contains two RCK domains harboring high-affinity calcium-binding sites known as the "calcium bowl" as well as magnesium-binding sites, both of which cooperatively mediate calcium/magnesium-dependent channel activation. Through this unique dual activation mechanism, KCNMA1 plays a central role in regulating vascular smooth muscle tone, controlling neurotransmitter release, maintaining auditory tuning in cochlear hair cells, and participating in innate immunity, among other critical physiological processes. Furthermore, the function of this protein is finely modulated by alternative splicing, phosphorylation, and other post-translational modifications, as well as by association with auxiliary β or γ subunits. Both loss-of-function and gain-of-function mutations in the KCNMA1 gene are closely associated with a spectrum of neurological disorders, including generalized epilepsy, paroxysmal dyskinesia, and ataxia.
