Background
Interleukin-4 (IL-4) is a 15–20 kDa, glycosylated, four-α-helical cytokine secreted primarily by Th2-polarized CD4⁺ T cells, mast cells, basophils and eosinophils that signals through a heterodimeric receptor complex composed of the IL-4Rα chain paired with either the common γ chain (type I, hemopoietic cells) or the IL-13Rα1 chain (type II, non-hemopoietic cells) to activate JAK1/3–STAT6 and IRS1/2 pathways, thereby driving commitment of naïve CD4⁺ T cells to the Th2 lineage, promoting immunoglobulin class-switching to IgG1 and IgE in B cells, inducing alternative activation (M2) of macrophages that orchestrate tissue repair and fibrosis, stimulating mucus production and smooth muscle remodeling in airway epithelium, and acting as a central mediator of allergic inflammation, asthma pathogenesis and resistance to helminth parasites, while also exerting context-dependent anti-inflammatory effects that limit Th1/Th17 responses and autoimmune damage.
