Background
The IL-6 receptor (IL-6R/CD126) is a type-I cytokine receptor formed by an 80 kDa ligand-binding subunit (IL-6Rα) and a 130 kDa signal-transducing subunit (gp130). gp130 is ubiquitous, whereas IL-6Rα is confined mainly to hepatocytes and immune cells. IL-6 signaling occurs via two routes: classical signaling through membrane-bound IL-6Rα and trans-signaling mediated by soluble IL-6R (sIL-6R) generated through proteolytic cleavage or alternative splicing; sIL-6R enables IL-6 to activate gp130 on cells that lack membrane IL-6Rα, thereby broadening its biological impact. Dysregulation of this pathway drives chronic inflammation (e.g., rheumatoid arthritis, Crohn’s disease) and promotes malignancies such as multiple myeloma, making the receptor a validated therapeutic target—anti-IL-6R antibodies have shown clinical efficacy. The extracellular region of IL-6Rα contains conserved Ig-like and fibronectin type-III domains bearing the WSXWS motif, underscoring its functional importance across species despite minor species-specific processing differences.
