Background

IFIH1 (Interferon Induced Helicase C Domain Containing Protein 1), also known as MDA5 (Melanoma Differentiation-Associated Gene 5), is a key intracellular retinoic acid-inducible gene I-like receptor (RLR) that specifically recognizes double-stranded RNA (dsRNA) produced during viral replication in the cytoplasm, serving as a sentinel in antiviral innate immunity. Upon viral invasion, IFIH1 binds dsRNA via its helicase domain, subsequently recruiting the downstream adaptor protein MAVS and activating TBK1/IKKε kinases, which then phosphorylate IRF3 and IRF7 to induce the production of type I interferons (such as IFN-α/β) and pro-inflammatory cytokines, launching a broad antiviral immune response. However, dysregulation of IFIH1 function can lead to immune imbalance: its gain-of-function mutations (such as p.A946T) cause hyperactivation of the type I interferon pathway, leading the body to misrecognize self-RNA as viral components, thereby triggering rare autoinflammatory diseases such as Aicardi-Goutières syndrome and Singleton-Merten syndrome; conversely, certain polymorphic variants (such as rs1990760) have been shown to be significantly associated with the risk of common autoimmune diseases including type 1 diabetes and systemic lupus erythematosus, suggesting that IFIH1 plays a delicate regulatory role in maintaining immune tolerance while activating antiviral defenses. Therefore, IFIH1 has become an important drug target for the treatment of autoinflammatory diseases and viral infections.
