Background
Sclerostin, encoded by SOST, is a secreted glycoprotein predominantly produced by osteocytes and a key negative regulator of bone formation. It binds LRP5/6 to antagonize canonical Wnt/β-catenin signaling, thereby suppressing osteoblast differentiation, proliferation, and matrix mineralization. Genetic studies of sclerosteosis and van Buchem disease, caused by SOST loss-of-function mutations, revealed a striking high-bone-mass phenotype, establishing sclerostin as a critical therapeutic target. Preclinical and clinical studies demonstrated that sclerostin inhibition, notably via romosozumab, rapidly increases bone formation and reduces fracture risk. Emerging research further implicates sclerostin in vascular calcification, chronic kidney disease, and cancer-associated bone metastasis, suggesting broader roles beyond skeletal homeostasis.
