Background

Histone H3.3 G34W is a histone variant generated by a mutation in the H3F3A gene, specifically involving a substitution of the normal glycine (G) with tryptophan (W) at the 34th amino acid residue of histone H3.3. This mutation serves as a highly specific molecular marker for Giant Cell Tumor of Bone (GCTB), detectable in approximately 90% of cases, making it a critical diagnostic tool for distinguishing GCTB from other histologically similar bone tumors. Mechanistically, H3.3 G34W functions as an "oncohistone" that disrupts H3K36 methylation mediated by the SETD2 enzyme, leading to widespread epigenetic dysregulation that impairs the normal differentiation of mesenchymal stem cells and drives tumorigenesis. Recent studies have further revealed that this mutation causes defects in DNA double-strand break repair—particularly in the non-homologous end joining pathway—resulting in genomic instability. However, this defect may simultaneously activate the cGAS/STING immune pathway, offering new potential strategies for combination radio-immunotherapy approaches.
