Background
HEG1 also known as the "Heart of Glass" homolog, is a single-pass transmembrane protein primarily localized at cell junctions. Its most core function is to serve as a receptor component of the CCM signaling pathway, which is critical for the formation and integrity of the heart and blood vessels. The HEG1 protein has a unique structure, featuring multiple epidermal growth factor (EGF)-like domains in its extracellular region, responsible for mediating intercellular signaling and adhesion, while its intracellular tail can form complexes with proteins such as KRIT1 and RAP1, collectively regulating the Rho protein signaling pathway to stabilize endothelial cell junctions and maintain vascular barrier function. During embryonic development, HEG1 is essential for cardiac morphogenesis, angiogenesis, and lymphatic development. Its dysfunction is not only associated with cardiovascular diseases such as congenital heart defects and vascular malformations, but recent studies have also found that its aberrant expression in tumors like hepatocellular carcinoma is closely linked to poor prognosis.
