Background

GPRC5D (G protein-coupled receptor class C group 5 member D) is an orphan Class C GPCR encoded by the GPRC5D gene on chromosome 12p13. The core transmembrane protein comprises ~308 residues, featuring a short N-terminal extracellular region (20–50 aa) and lacking the large Venus flytrap domain typical of classical Class C GPCRs.

Structurally, it forms a symmetric homodimer via TM4-TM4/5 interfaces, stabilized by cholesterol molecules. It has an unusually short ECL2, creating an exposed shallow extracellular pocket, and displays weak constitutive Gi/o activity in an inactive-like conformation.

GPRC5D is highly and specifically overexpressed on malignant plasma cells in multiple myeloma (MM), with expression independent of BCMA. High GPRC5D levels correlate with shorter progression-free survival.

Therapeutically, Talquetamab (FDA-approved bispecific GPRC5D/CD3 T-cell engager) and GPRC5D-targeted CAR-T cells have shown potent preclinical/clinical activity, with no significant on-target/off-tumor toxicity. Additionally, LM-305, a GPRC5D-targeting ADC developed by LaNova Medicines (licensed to AstraZeneca), is in Phase I/II trials for relapsed/refractory MM.
