Background
Cytokeratin 18 (CK18) is an intermediate-filament structural protein restricted to epithelial cells that becomes a specific biomarker of apoptosis when caspases 3, 6, 7 and 9 cleave it between residues 387–396, generating the soluble neo-epitope termed “M30”; this stable fragment is released into blood or culture supernatant only during programmed cell death, can be quantified by ELISA or immunohistochemistry with the M30 antibody, and thus serves as a non-invasive pharmacodynamic read-out of epithelial apoptosis in chronic liver diseases, NAFLD, viral hepatitis and cancer therapy monitoring.
