Background
Cardiac Troponin T (cTnT) is a highly specific regulatory protein found in myocardial cells and serves as a critical biomarker for diagnosing cardiac injury. Under normal physiological conditions, cTnT levels in the blood are negligible. However, when myocardial cells are damaged or undergo necrosis, the structural integrity of the cell membrane is compromised, causing cTnT to be released into the systemic circulation.
Clinically, cTnT is regarded as the "gold standard" for the diagnosis of Acute Myocardial Infarction (AMI). Levels typically begin to rise 3 to 4 hours after the onset of symptoms, providing an early indication of ischemia. A distinct advantage of cTnT is its prolonged elevation; it can remain detectable in the serum for 10 to 15 days. This extended window is particularly valuable for diagnosing late-presenting infarctions that might be missed by shorter-lived markers.
Beyond acute infarction, cTnT has significant prognostic value. It aids in risk stratification for patients with unstable angina, helps evaluate the efficacy of thrombolytic therapy through dynamic monitoring, and assists in detecting micro-infarctions following cardiac surgery. Additionally, elevated cTnT is a crucial indicator for assessing cardiovascular risks in patients with renal failure and diagnosing conditions such as myocarditis and heart failure.
