Background

CRMP5 (Collapsin Response Mediator Protein 5, also known as CV2) is the fifth member of the collapsin response mediator protein family, sharing only about 50% sequence homology with other family members, making it relatively unique. This protein is primarily highly expressed in the nervous system during embryonic development and early postnatal stages, localizing to the cytoplasm and neurites, where it participates in regulating neuronal differentiation and cytoskeletal rearrangement. Functionally, it antagonizes other family members (particularly the neurite-promoting CRMP2) by binding to tubulin and inhibiting microtubule polymerization, thereby negatively regulating dendritic growth and formation, which is crucial for the construction of neural networks. Additionally, CRMP5 holds unique pathological significance: it has been identified as a major autoantigen in paraneoplastic neurological syndromes. In approximately 80% of patients with small cell lung cancer or thymoma, tumor-induced anti-CRMP5 autoantibodies attack the nervous system, leading to hyperexcitability of sensory neurons in the dorsal root ganglia and consequently causing refractory neuropathic pain. Furthermore, in tumors such as glioblastoma, high expression of CRMP5 can activate the Akt signaling pathway by inhibiting the degradation of Notch receptors, thereby driving the proliferation of cancer stem cells, making it also considered a potential cancer biomarker and prognostic factor.
