Background

CLPP is a highly conserved serine protease located in the mitochondrial matrix that serves as a core component in maintaining mitochondrial protein homeostasis. Its function depends on binding with the ATPase chaperone CLPX to form the CLPXP complex, in which CLPX recognizes and, using ATP hydrolysis energy, unfolds damaged or misfolded protein substrates before translocating them into the proteolytic chamber formed by the double heptameric barrel structure of CLPP, where they are degraded into small peptides—thereby eliminating toxic protein aggregates within mitochondria. This system is broadly conserved across species (for instance, human CLPP consists of 277 amino acids and is localized at chromosomal region 19p13.3). Beyond its physiological functions, aberrations in CLPP are closely linked to various diseases: germline mutations can cause Perrault syndrome type 3, a rare autosomal recessive disorder characterized by sensorineural hearing loss and ovarian insufficiency in females; in cancer biology, CLPP is overexpressed in multiple tumors including multiple myeloma and breast cancer, correlating with poor prognosis, which positions it as a promising anticancer target. Small-molecule agonists (such as ONC201) that induce its hyperactivation can lead to mitochondrial dysfunction and tumor cell apoptosis, while inhibition of its activity can also effectively kill myeloma cells and overcome drug resistance.
