Background

Chitinase-3-like protein 1 (CHI3L1, also named YKL-40) is a valuable circulating biomarker for in vitro diagnostics, reflecting inflammation, extracellular matrix remodeling, fibrosis and tumor progression. Secreted by macrophages, fibroblasts and malignant cells, it can be quantified in serum, cerebrospinal fluid and urine via ELISA, chemiluminescence and other immunoassays, supporting convenient, minimally invasive detection.

Its strongest clinical value lies in noninvasive liver fibrosis staging and hepatocellular carcinoma surveillance, where serum CHI3L1 correlates closely with fibrosis severity and outperforms several routine noninvasive indices in some cohorts. It also aids risk stratification in interstitial lung disease, neurodegeneration and multiple solid tumors, with elevated levels linked to advanced stage and poorer prognosis.

Yet CHI3L1 lacks disease specificity: it rises across many inflammatory, metabolic and fibrotic conditions, so it works best in multi-marker panels rather than as a standalone test. For IVD development, CHI3L1 offers a stable, measurable analyte to fill unmet needs for fibrosis and inflammatory disease monitoring. Robust assay precision and standardized cutoffs will be critical to translate its broad biological relevance into reliable clinical decision support.
