Background
CFAH, or Complement Factor H, is a critical regulatory glycoprotein of the alternative complement pathway that plays an essential role in maintaining immune homeostasis by distinguishing host cells from pathogens and preventing excessive complement activation on self-tissues. Synthesized primarily in the liver and circulating at high concentrations in plasma, CFAH exerts its protective functions through cofactor activity for Factor I-mediated cleavage of C3b and by accelerating the decay of the C3 convertase (C3bBb), thereby limiting amplification of the complement cascade on host surfaces rich in polyanions like sialic acid and glycosaminoglycans. Genetic polymorphisms, particularly the Y402H variant in the CFH gene, and mutations affecting its functional domains are strongly associated with susceptibility to age-related macular degeneration, atypical hemolytic uremic syndrome, and membranoproliferative glomerulonephritis, underscoring its vital importance in both innate immunity and the pathogenesis of complement-mediated diseases.
