Background

CD46, also known as complement regulatory protein or membrane cofactor protein (MCP), is a type I transmembrane glycoprotein widely expressed on human cell surfaces. Its structure comprises four complement control protein repeats (CCPs), a serine/threonine-rich region, a transmembrane domain, and a cytoplasmic tail, with multiple isoforms generated through alternative splicing. The core function of CD46 lies in its role as a key regulator of the complement system, where it serves as a cofactor for the proteolytic cleavage of C3b/C4b by factor I, thereby protecting host cells from complement-mediated cytolysis. Additionally, CD46 acts as a cellular receptor for various pathogens, including measles virus and adenovirus, and participates in modulating T cell metabolism and immune responses. In recent years, CD46 has emerged as a promising therapeutic target in oncology due to its overexpression in multiple solid tumors—such as prostate, bladder, and multiple myeloma—spurring the clinical development of antibody-drug conjugates (ADCs), targeted alpha-particle radiotherapy, and theranostic strategies, all demonstrating favorable therapeutic potential.
