Background

CD40L (also known as CD154 or gp39, encoded by TNFSF5) is a member of the tumor necrosis factor superfamily expressed as a homotrimeric transmembrane glycoprotein on activated T cells, platelets, and vascular endothelial cells, with an extracellular TNF homology domain that can be cleaved by metalloproteinases to release soluble sCD40L. Through binding to its receptor CD40, CD40L serves as a central regulator of adaptive immunity by facilitating T-B cell crosstalk, driving antibody class switching and germinal center formation, activating dendritic cell maturation, and enhancing cytotoxic T lymphocyte responses. Clinically, sCD40L serves as an independent biomarker for atherosclerosis, acute coronary syndromes, and thrombotic events; CD40L deficiency causes X-linked hyper-IgM syndrome and other immunodeficiencies, whereas its aberrant activation is implicated in autoimmune diseases, transplant rejection, and tumor immune evasion, making the CD40/CD40L axis a critical therapeutic target for cardiovascular and immune-related disorders.
