Background
CD146, also known as melanoma cell adhesion molecule (MCAM), is a transmembrane glycoprotein involved in cell–cell adhesion, endothelial integrity, and lymphocyte trafficking across the blood–brain barrier. Its soluble form, sCD146, is increasingly recognized as a clinically useful cerebrospinal fluid (CSF) biomarker reflecting neuroinflammation and barrier dysfunction.
In inflammatory demyelinating diseases, elevated CSF sCD146 distinguishes active multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD) from non-inflammatory controls, and correlates with disease activity and lesion burden, aiding differential diagnosis and relapse monitoring. In autoimmune encephalitis (AE) spectrum disorders, increased sCD146 mirrors intrathecal inflammation and blood–brain barrier breakdown, supporting early diagnosis when antibody testing is delayed or negative. In central nervous system infections—including viral, bacterial, and tuberculous meningitis—sCD146 rises in parallel with meningeal inflammation and may help differentiate infectious from aseptic etiologies and assess treatment response.
Thus, CD146 serves as a valuable adjunctive diagnostic marker, integrating mechanistic insight with clinical utility across a broad spectrum of neuroinflammatory and infectious CNS diseases.
