Caspase-4 is a key protein belonging to the inflammatory caspase family, serving as a direct sensor of intracellular bacterial lipopolysaccharide (LPS) in the human innate immune system. When Gram-negative bacteria invade the host cell cytoplasm, Caspase-4 recognizes and binds LPS via its CARD domain, followed by self-oligomerization and activation. It then cleaves and activates the downstream protein Gasdermin D (GSDMD), which forms pores in the cell membrane and induces an inflammatory form of cell death known as "pyroptosis," while concurrently promoting the maturation and release of pro-inflammatory cytokines (such as IL-1β and IL-18), thereby restricting pathogen replication and spread. However, hyperactivation of Caspase-4 is also closely associated with various inflammatory diseases, including sepsis, inflammatory bowel disease, and gout, and thus specific inhibitors of Caspase-4 are being actively developed as potential anti-inflammatory agents.
