Background

BRD4 (bromodomain-containing protein 4) is a nuclear protein belonging to the bromodomain and extra-terminal domain (BET) family, encoded by the BRD4 gene on human chromosome 19. This protein contains two highly conserved bromodomains (BD1 and BD2) and an extra-terminal domain (ET), enabling it to specifically recognize and bind acetylated histones and non-histone proteins. By recruiting positive transcription elongation factor b (P-TEFb) and other transcriptional regulatory complexes, BRD4 functions as a core coactivator in the process of transcriptional elongation. Under physiological conditions, BRD4 broadly regulates the expression of genes involved in cell cycle progression, proliferation, and differentiation, thereby maintaining genomic stability. However, in various malignancies (such as acute myeloid leukemia, triple-negative breast cancer, NUT midline carcinoma, etc.), BRD4 frequently forms BRD4-NUT fusion proteins through chromosomal translocation or is aberrantly recruited to super-enhancer regions, driving sustained high expression of oncogenes (e.g., MYC, BCL2) and promoting tumor progression. Recent studies have demonstrated that the functions of BRD4 extend beyond transcriptional regulation; it also participates in DNA damage repair, modulates autophagy, and regulates inflammatory responses. Meanwhile, small-molecule inhibitors targeting BRD4 (e.g., JQ1), which competitively block its binding to acetylated histones, have exhibited significant antitumor activity in numerous preclinical models. Furthermore, BRD4 plays an important role in cardiac development and maintenance of vascular endothelial function, and its expression dysregulation is closely associated with cardiovascular diseases such as cardiac hypertrophy and atherosclerosis. In summary, BRD4 serves as a critical hub linking epigenetic modifications and transcriptional regulation, holding great significance in both normal development and disease pathogenesis.

Counterstained with hematoxylin. Heat mediated antigen retrieval with Tris/EDTA buffer pH9.0 was performed before commencing with IHC staining protocol.
