ITGAV (integrin subunit alpha V) and ITGB6 (integrin subunit beta 6) are heterodimeric cell adhesion receptors that non-covalently pair to form the αvβ6 integrin, the only integrin in which β6 exclusively associates with αv. Normally expressed at low levels in healthy epithelia, αvβ6 is markedly upregulated in multiple solid tumors and fibrotic diseases. Functionally, αvβ6 activates latent TGF-β, thereby driving tumor invasion, epithelial remodeling, and immune evasion. Clinically, αvβ6 has emerged as a promising therapeutic and diagnostic target. The dual αvβ6/αvβ1 inhibitor bexotegrast has shown acceptable safety and antifibrotic trends in phase II INTEGRIS-PSC trials for primary sclerosing cholangitis. PET/CT imaging targeting αvβ6 enables staging and treatment guidance in esophageal cancer, while ITGB6 inhibition stimulates potent antitumor immunity in head and neck squamous cell carcinoma and pancreatic adenocarcinoma. Moreover, ITGAV serves as a pan-cancer diagnostic, immunological, and prognostic biomarker. These findings support further development of αvβ6-targeted antibody-drug conjugates, radionuclide theranostics, and combination immunotherapies for precision oncology.
