Background

Interleukin-21 receptor (IL-21R) is a type I single-pass transmembrane glycoprotein belonging to the class I cytokine receptor family. Its structure includes an extracellular immunoglobulin-like domain, a conserved WSXWS motif (responsible for proper folding and transport), and an intracellular Box 1 motif (for JAK kinase binding). It must form a functional receptor complex by associating with the common gamma chain (γc). IL-21R is predominantly expressed on lymphohematopoietic cells such as B cells, T cells, and NK cells. Upon binding to its ligand IL-21, it regulates humoral and cellular immunity through signaling pathways such as JAK-STAT, including promoting B cell differentiation into plasma cells and antibody production (e.g., IgG1), enhancing the cytotoxic functions of NK cells and CD8⁺ T cells, and modulating T cell differentiation (e.g., Th17 and regulatory T cells). Abnormal activation of the IL-21R signaling pathway is closely associated with autoimmune diseases (such as systemic lupus erythematosus and allergic dermatitis) and lymphomas (such as diffuse large B-cell lymphoma). By driving the production of pathogenic autoantibodies and excessive activation of immune cells, it contributes to disease pathogenesis, making it an important therapeutic target for autoimmune disorders and hematological malignancies.
