Background

IL-2 (interleukin-2) is a four-helix bundle cytokine that signals through a high-affinity receptor complex comprising IL-2Rα, IL-2Rβ, and the common γ-chain (γc), activating the JAK1–JAK3 heterodimer and STAT5 pathway to regulate T cell development, differentiation, memory formation, and immune tolerance. Structurally, IL-2 forms a ternary complex with its receptor, and engineering of the binding interface enables biased signaling. Functionally, IL-2 exerts dual regulatory effects on effector T cells and regulatory T cells (Tregs). Clinically, low-dose IL-2 is used to expand Tregs in autoimmune diseases such as systemic lupus erythematosus, while engineered strategies—including biased IL-2 mutants, PD-1-cis IL-2R agonists, and orthogonal IL-2/IL-2Rβ systems—selectively activate tumor-specific CD8+ T cells, opening new avenues for cancer immunotherapy.
