Background

Brain-derived tau (BD-tau) is a specific isoform of the tau protein originating from the central nervous system (CNS), now used as a highly precise blood-based biomarker for Alzheimer’s disease and other neurodegenerative conditions. It accurately measures brain-specific pathology, distinguishing it from peripheral tissue-derived tau, and correlates strongly with cerebrospinal fluid (CSF) markers of Alzheimer's, making it a valuable tool for early diagnosis and treatment monitoring. Unlike total tau (t-tau), which is found throughout the body, BD-tau specifically detects neuronal damage within the brain, offering greater diagnostic specificity for Alzheimer's and related dementias. It is measured in blood (plasma/serum) using ultra-sensitive technology like Quanterix’s Simoa or Beckman Coulter's automated assays. Elevated BD-tau levels in plasma indicate Alzheimer's-type neurodegeneration, correlate with amyloid plaque and tangle accumulation, and can aid in tracking treatment effects and disease progression.
