Background

Enfortumab Vedotin (EV), as a Nectin-4-targeting antibody-drug conjugate (ADC), derives its core value from the precise delivery of a microtubule inhibitor to tumor cells. Beyond exerting direct cytotoxicity through Nectin-4 targeting, EV synergizes with immune checkpoint inhibitors to enhance T cell-mediated antitumor immune responses. The regimens of EV monotherapy and its combination with pembrolizumab have fundamentally revolutionized the treatment landscape of urothelial carcinoma, significantly prolonging overall survival in the first-line setting and demonstrating promising potential for deep downstaging and prognostic improvement in the perioperative setting. However, its distinctive cutaneous and peripheral neurotoxicities warrant close monitoring and meticulous management in clinical practice.
