Background

IL-6R (Interleukin-6 receptor) is a pivotal cytokine receptor that exists in both membrane-bound (mIL-6R) and soluble (sIL-6R) forms, mediating the biological effects of Interleukin-6 (IL-6) by associating with the signal transducer gp130. Its core functionality is defined by two distinct signaling modes: classical signaling, mediated by mIL-6R, is largely restricted to hepatocytes and immune cells, regulating physiological processes such as tissue regeneration and anti-inflammatory responses; in contrast, trans-signaling, mediated by sIL-6R, binds with IL-6 to target virtually any cell expressing gp130 (such as endothelial and smooth muscle cells), vastly expanding the scope of IL-6 activity and serving as the primary driver of chronic inflammation and various pathological conditions, including rheumatoid arthritis and cytokine storms. Consequently, IL-6R is regarded as a critical hub connecting physiological regulation with pathological damage, and monoclonal antibodies targeting it (such as tocilizumab) have become a vital strategy for treating various immune-mediated inflammatory diseases by blocking both signaling pathways.
