Background
Interleukin-6 (IL-6) is a pivotal pro-inflammatory cytokine with multifaceted biological functions, primarily secreted by activated macrophages, T cells, fibroblasts, and endothelial cells in response to infection, trauma, or stress; it exerts its effects by binding to either membrane-bound IL-6 receptors (mIL-6R) or soluble receptors (sIL-6R) on target cells, thereby activating downstream signaling pathways such as JAK/STAT (predominantly JAK1/2-STAT3), MAPK, and PI3K/Akt. Through these mechanisms, IL-6 induces the liver to produce acute-phase proteins like C-reactive protein (CRP) and fibrinogen, while extensively regulating immune cell differentiation (e.g., promoting Th17 cell generation and inhibiting regulatory T cells), hematopoietic stem cell proliferation, bone metabolism, and neuroendocrine interactions. Notably, IL-6 exhibits a significant "double-edged sword" characteristic: while appropriate levels are crucial for host defense and tissue repair, sustained or excessive elevation is closely linked to the pathogenesis of rheumatoid arthritis, cytokine storms, various autoimmune diseases, and malignancies, making it a critical clinical inflammatory biomarker
