Background

GUCY2C (guanylyl cyclase C) is a transmembrane receptor protein predominantly expressed in intestinal epithelial cells. Upon activation by endogenous ligands guanylin and uroguanylin, it catalyzes GTP to generate cGMP, thereby regulating intestinal fluid secretion, ion transport, and mucosal barrier homeostasis. In colorectal cancer, GUCY2C exhibits tumor-specific overexpression in over 95% of cases while remaining virtually absent in extra-intestinal tissues, making it an ideal tumor-specific target. Clinically, GUCY2C serves not only as a diagnostic and prognostic biomarker for colorectal cancer but also as a core target for targeted therapeutics—from the first-generation antibody-drug conjugate Indusatumab (TAK-264) to second-generation ADCs (TAK-164), as well as bispecific antibodies and CAR-T cell therapies. GUCY2C-targeted treatments are currently undergoing a critical transition from proof-of-concept to clinical translation, offering novel directions for precision therapy in metastatic colorectal cancer and other gastrointestinal malignancies.
