Background
Dupilumab (brand name Dupixent) is a fully human monoclonal antibody (IgG4 type) that inhibits the type 2 inflammatory signaling pathway mediated by two key cytokines by blocking the shared receptor component of interleukin-4 (IL-4) and IL-13. Administered by subcutaneous injection, the drug has been approved by the U.S. FDA since its first approval in 2017 for multiple type 2 inflammation-related diseases, including atopic dermatitis, asthma, chronic rhinosinusitis with nasal polyps, eosinophilic esophagitis, prurigo nodularis, bullous pemphigoid, and allergic fungal rhinosinusitis. Clinical data show that dupilumab has a relatively rapid onset of action, significantly improving pruritus and skin lesions; most patients with atopic dermatitis experience symptom relief within days to weeks of treatment, and long-term treatment can achieve essentially clear skin in a substantial proportion of patients. Its overall safety profile is favorable, with the most common adverse reactions being injection-site reactions and conjunctivitis; a small number of patients may experience keratitis, arthralgia, or transient peripheral blood eosinophilia, but the incidence of serious adverse events is low. It should be noted that the drug is not indicated for the relief of acute asthma attacks or acute bronchospasm, and pre-existing systemic or inhaled corticosteroids should not be abruptly discontinued at the start of treatment but should be gradually tapered under medical supervision.
